To set an appointment or to ask me any questions, please call 706-389-8161.

For your first appointment, please bring with you:

  • Completed Counseling Paperwork
  • Insurance Card (if you are planning to use insurance)
  • Picture ID
  • Session payment (cash or check are acceptable, I DO NOT accept credit cards

Initial Paperwork

You will need to complete the Intake Paperwork before the first session.  It should take approximately 20 minutes to complete and contains important information about confidentiality, privacy, cancellations, and fee collection and we will discuss it at length during the first meeting.

Costs

The first session you have will be considered an intake to help assess your concerns, goals, and to see if we might be a good fit to work together.  Intakes are 60 minutes and cost $150.  Individual sessions are approximately 55 minutes and cost $140.  In an effort to make sessions more affordable, I have a few slots for those who cannot afford my full fee. Please let me know if you require a sliding scale fee.

Insurance Information

I accept Blue Cross/Blue Shield and the UGA student insurance (United-Student Division) and will file these insurance claims for you.  I am an out-of-network provider for all other insurance companies, which means I can give you a receipt with the necessary information provided so that you could file for direct reimbursement from the insurance company.

If you are using insurance, you would first want to check to see if you have mental health/ behavioral health coverage.  Check with your insurance company carefully and be sure to ask them the following questions:

  1. Do I have mental health/behavioral health benefits?
  2. Do I need pre-authorization?
  3. How much does my plan cover for an out-of-network provider? 
  4. What is my co-pay for a provider in-network?
  5. What is my deductible and has it been met?
  6. Is approval/referral required from my primary care physician?

Five Points Location

Five Points Acupuncture is inside a house on South Milledge close to the Milledge exit of the loop.  It is on the same side of the street and close to Entourage and George's Low Country Grill.

The Athens Transit bus Route 9 drops off directly across the street at the Shops of South Athens stop for the Macon Highway/Five Points route.

The Athens Transit bus Route 14 drops off close to the building at either the Carousel Village apartments or the Raintree Apartments on the East campus/South Milledge route.
